Documentation Guidelines for Learning Disabilities

Determination of a current learning disability (LD) is made by the Learning Disabilities Specialist (LDS).

Students must provide written age-based standard scores generated within the past three years. The LDS will study the scores. The assessment may be documented by an outside source such as a licensed psychologist, school psychologist, learning disability specialist, or neurologist. After that the student will undergo an interview with the LDS. The LDS makes the final determination as to whether and how much assessment is warranted to complete the assessment.

Documentation must include the following:

  • Date of assessment
  • Instruments used
  • Relevant psychosocial history
  • Age-based standard scores for indices and scaled scores for subtests
  • Interpretive discussion to include:
    • Alternative explanations for chief complaints
